About Us
Created by Erin Fay-Azzato MS OTR/L, Home Safe Home has become the premier resource for keeping your loved ones safe at home. Erin, a Masters graduate of Tufts University, has over 20 years of personal and professional experience in various rehabilitation settings. Throughout the years, Erin has recognized the need for greater safety and fall prevention in the home. She realizes that each of her clients wish to live independently and safely in their own home. Along with providing a comprehensive safety assessment, Erin is able to identify potential risks, make modifications, and provide safety training. Her services give individuals the confidence in which to live, and move safely throughout their own home.

Erin and her family experienced
first hand the need for this specialized service when her father suffered a
major and debilitating stroke at the young age of 50. After multiple safety
modifications were made to their home, Erin’s father was able to enjoy the
remaining 15 years of his life with his family in the comfort of his own home.
Overview of Services
Functional Home Assessment
- Client interview complete with past medical history & current physical abilities
- Comprehensive Safety Assessment performed
- Risks identified and Modifications recommended
Contractor Coordination
- An initial in-home meeting with a licensed contractor
- Communication throughout the modification process
- Secondary review upon completion of work performed to ensure client satisfaction
Physician Coordination
- Communicate current medical status to Physician
- Obtain any necessary medical orders for equipment from Physician
Medical Equipment Coordination
- Coordinate with a medical supply company to obtain recommended equipment
Teaching & Education
- A licensed therapist provides a follow-up teaching that provides a client with the tools and confidence to live in and move safely throughout the home
